چکیده
1 regurgitation or stenosis. It occurs in 1 percent of congenital heart defects. It is associated with either atrial septal defect or patent foramen ovale through which shunting of blood occurs. Haemodynamic derangement depends upon status of tricuspid valve displacement and right to left shunt. Hence, clinical presentation varies from congestive cardiac failure in childhood to incidental diagnosis in adulthood. Though, pregnancy is well tolerated in Ebstein's anomaly, such uncorrected maternal congenital heart defect may have deleterious effects on the health of both the mother and her offspring. Case Report 20 yr old primigravida, 36 weeks of gestation was posted for emergency LSCS for meconeum stained amniotic fluid. In third trimester she was presented with dyspnoea on exertion NYHA grade II and diagnosed as Ebstein anomaly with patent foramen ovale. After cardiac evaluation, she was started on T. Lasix & T. Digitoxin. On clinical examination, pulse120/min, occasional missed beats, BP120/90 mmHg RUL, CVS systolic murmur at tricuspid area, RR24/min, RSclear. There were no signs of congestive cardiac failure such as raised JVP, pitting pedal oedema, hepatomegaly. Her investigations were normal except Hb15.5 (polycythaemia), ECG showing sinus tachycardia with occasional VPCs. 2D ECHOEbstein anomaly of tricuspid valve, TR, dilated RA & RV, PFO, bidirectional shunt, predominantly L R shunt, no PH, normal LV systolic function. ABG on air showed normal pH (7.4), pCO2 (27), but pO2 (58), SO2 (91%) was on lower side and

Anaesthesia for caesarean section in a pre-eclamptic patient with Ebstein's anomaly.
Anaesthetic management for Caesarean section in a pre-eclamptic patient with Ebstein's anomaly and a right to left intracardiac shunt was accomplished under general anaesthesia. Air bubbles in the venous line were meticulously eliminated to prevent paradoxical air emboli. A rapid sequence induction using standard doses of thiopentone and succinylcholine did not result in loss of consciousness a...
متن کاملCommon Complaints Associated with Caesarean Section during Spinal Anaesthesia
Over 20% of deliveries require a caesarean section for the birth of the baby. Caesarean deliveries are performed as an elective or an emergency procedure under General, Spinal, Epidural or combined spinal epidural anaesthesia. The majority of caesarean sections are done under spinal anaesthesia now. Spinal Anaesthesia is preferred for caesarean section because it is simple to perform and is eco...

PURPOSE This is the first report describing combined spinal epidural anaesthesia for labour and unexpected Caesarean section in a patient with mitral and aortic stenosis and insufficiency. CLINICAL FEATURES The patient was a 30-yr-old GIPO with a history of rheumatic fever. She had moderate stenosis and insufficiency of the mitral and aortic valves. Combined spinal epidural anaesthesia was us...

Good multidisciplinary communication is crucial to the safe management of women requiring non-elective Caesarean section. Anaesthetists should participate actively in resuscitation of the fetus in utero; relief of aortocaval compression is paramount. Epidural top-up with levobupivacaine 0.5% is the anaesthetic of choice for women who have been receiving labour epidural analgesia. If epidural to...

PURPOSE To report a case of peripartum dilated cardiomyopathy presenting for emergency caesarean section, this was successfully managed with Epidural Anaesthesia. CLINICAL FEATURES A parturient suffering from idiopathic peripartum cardiomyopathy (E.F. 18%) was brought for an emergency caesarean section. Epidural anaesthesia was performed and 2% Lignocaine with adrenaline total 13 ml was injec...
